Illinois CPDU Information
When you register at the conference (or when you pick up your materials if you’ve preregistered) please ask for an Illinois Educator packet. This packet will include your Illinois State Board of Education Evaluation and Evidence of Completion Forms and a schedule of clinics and concerts that will be used to verify your attendance at the individual sessions. At the completion of each session that you attend, ask the usher to stamp this schedule to verify your attendance at that session.
When you leave The Midwest Clinic, you will need to report to the registration desk with your Evaluation and Evidence of Completion forms and your schedule. A Midwest staff member will then add the total number of verified hours on your schedule to your Evidence of Completion form in the space marked Duration. At that time we will also collect your Evaluation form, which we will retain for at least three years.
You may then submit your verified schedule and Evidence of Completion Form to your Local Professional Development Committee. Your LPDC will determine how many CPDU’s to credit for your participation based on the amount of time spent in the sessions you attended.
