2016 Registration Fees & Conference Dates

Registration Fees 

Please note:  In celebration of our 70th Anniversary, we're keeping registration rates for 2016 the same as 2015!

Early Bird Director Rate:  $120 (available now until 9/23)*
Pre-Registration Director Rate:  $150 (available until 12/2)
On-Site Director Rate:  $175
First Time Attendee (Director) Rate:  $85 (available until 12/2)
College Student Pre-Registration Rate:  $50 (available until 12/2)
College Student On-Site Registration Rate:  $75
Spouse Rate:  $40
Chaperone Rate:  $25
Primary/Secondary Student Rate:  $25 (must be connected to a paid Director registration).
Shuttle Pass:  $25

NEW for 2016:  Gala Dinner/Entertainment Tickets for Friday, Dec. 16 -  $50 each




*Cancellation and Refund Policy

Early Bird rate is non-negotiable and NON-REFUNDABLE. Any cancellations received after payment has been made will result in a fee equal to 100% of advance payment.

If you need to cancel your registration, you must do so in writing. Please email your cancellation to: info@midwestclinic.org. If we receive your cancellation ON or BEFORE December 2, 2016, we will deduct 35% off your payment for processing charges and credit the balance to your credit card. There will be NO REFUNDS issued for cancellations after the Dec. 2 deadline. This is non-negotiable.



*Payment Policy

Full payment or purchase order must be received 30 days after completing registration. If payment or purchase order is not received by this deadline, the registration will be cancelled.

Purchase Orders DO NOT constitute payment. All purchase orders must be paid in full prior to or upon arrival at the conference. If mailing the check, it must be received by December 2, 2016 otherwise payment will be due upon arrival at the conference.

** The option to pay by Purchase Order/Check will only be available until Nov. 23.  After this date, pre-registration must be paid by credit card.
